Important Notes:

1. Workplace/organizational Teams - An appointed team leader will only need to submit 1 application on behalf of their entire group.

2. Individual volunteers not associated with a workplace/organizational team are also invited to complete this application.

3. Both team leaders and individuals will be contacted about project assignment at least 2 weeks prior to the event for additional details.
